Forgetting to leave room for audio. This calculates total bitrate; if your encoder sets video and audio bitrate separately, subtract your audio bitrate (usually 128-320 kbps) first.
Setting bitrate too low for the resolution. Very low bitrates for high resolutions cause visible compression artifacts — check the Resolution Calculator for recommended minimums.
Most encoders use variable bitrate by default, which can cause some deviation. For a closer match, use a constant bitrate (CBR) setting in your export tool.
